Patient question: I have a round face specially when I smile, I believe its because my face is fatty. I look so fat on the photos when I smile.I want to have sleek cheeks and contoured face.I believe this will increase my self confidence.pls help... Dr. John Mesa's Answer:
Patients of asian background who have chubby cheeks ( that makes them look round face specially when smiling) usually have enlarged buccal fat pads. Buccal fat pads are defined pockets of fat located deep in the lower anterior cheeks. This pockets of fat are sometimes enlarged (specially in patients from asian background ) and are resistant to decrease with diet and exercise. Buccal fat removal is the plastic surgery procedure that safely removes the buccal fat pads. This procedure allows the face to achieve a permanent thinner appearance that doesn't look round when smiling. The procedure also allows to achieve a more chiseled facial look. RF (radio frequency) procedures that melt fat ( usually underneath the skin treated) are unlikely to "melt" or "dissolve" the buccal fat pads. The reason is because they are located very deep inside the cheeks, which makes them extremely difficult to be reached by RF delivered thought the skin. POTENTIAL PATIENT QUESTION: I am unsure of which one would be more suitable and give a more slimmer defined look to the face. Is it buccal fat of enlarged masseter muscles. I know for sure that my masseter muscles are enlarged as my dentist has told me this which I think is giving me a square jawline however I am also unsure if I have Botox in this are will it give the desired result? Any advice would be much appreciated? Thanks. DOCTOR MESA'S ANSWER:
To achieve a a more slimmer defined look to the face, very often not only one but several plastic surgery procedures are necessary to achieve such result. Buccal fat removal helps to correct the fullness of the ANTERIOR lower cheeks. In patients with high cheek bones and normal size masetter muscle this helps to achieve a chiseled V shape face. Botox to the masetter muscle decreases the fullness of the POSTERIOR lower cheek by shrinking the muscle. This complements the results of buccal fat removal in patients with wide jaw that would like to have a more V shape face. When patients have some heaviness of skin / soft tissue bellow the corners of the mouth, very often it means the cheek bones are a little bit weak. Therefore, unless they are enhanced, patient might bot be able to achieve a more more angular and v shaped face with just buccal fat removal. In this cases, I recommend my patients interested on buccal fat removal to undergo cheek bone enhancement with fillers like Voluma. This in conjunction with buccal fat removal and botox to masseter does help to achieve that angular V shaped facial look. John Mesa, MD Buccal Fat Removal Specialist UES New York City |
