POTENTIAL PATIENT QUESTION: I am unsure of which one would be more suitable and give a more slimmer defined look to the face. Is it buccal fat of enlarged masseter muscles. I know for sure that my masseter muscles are enlarged as my dentist has told me this which I think is giving me a square jawline however I am also unsure if I have Botox in this are will it give the desired result? Any advice would be much appreciated? Thanks. DOCTOR MESA'S ANSWER:
To achieve a a more slimmer defined look to the face, very often not only one but several plastic surgery procedures are necessary to achieve such result. Buccal fat removal helps to correct the fullness of the ANTERIOR lower cheeks. In patients with high cheek bones and normal size masetter muscle this helps to achieve a chiseled V shape face. Botox to the masetter muscle decreases the fullness of the POSTERIOR lower cheek by shrinking the muscle. This complements the results of buccal fat removal in patients with wide jaw that would like to have a more V shape face. When patients have some heaviness of skin / soft tissue bellow the corners of the mouth, very often it means the cheek bones are a little bit weak. Therefore, unless they are enhanced, patient might bot be able to achieve a more more angular and v shaped face with just buccal fat removal. In this cases, I recommend my patients interested on buccal fat removal to undergo cheek bone enhancement with fillers like Voluma.
